Driver Services Forms and Other Documents
Most Motor Vehicle forms must be printed, filled out manually and delivered or mailed to the address indicated on the form. Forms cannot be submitted online. Please follow any instructions carefully. See Help for assistance with viewing Portable Document Format (PDF) files.
Fill/Save forms – Most forms contain form fields that can be filled in on-screen. You may fill the form in on your computer, save and print the completed form. To save the typed in text in the PDF form, you must have Adobe Reader 7.03 or later. Be sure to sign it before delivering or mailing it to the address indicated on the form.

Mail renewal forms for regular Class D driver license
To be eligible to renew your Montana driver license by mail, you must either:
- Be a Montana resident who is temporarily outside the state and will not return prior to the expiration of your Montana driver license OR
- Live in a county that does not provide driver license services—Carter, Garfield, Golden Valley, Jefferson, Judith Basin, Madison, Petroleum, Prairie, Treasure and Wibaux.

Mail renewal forms for commercial driver license
To be eligible to renew a Montana commercial driver license by mail, you must either:
- Be a Montana resident who is temporarily outside the state and will not return prior to the expiration of your Montana driver license OR
- Live in a county that does not provide driver license services—Carter, Garfield, Golden Valley, Jefferson, Judith Basin, Madison, Petroleum, Prairie, Treasure and Wibaux.
Read Renewal by Mail instructions before filling out renewal forms. All three of the following forms are required to be completed and submitted, along with a photocopy of a valid Department of Transportation (DOT) medical card:
